To update details like your name, address, phone numbers or email address:
To maintain full GP services, including home visits, your new address must be within our catchment area. If your new address is outside of our catchment area, you may still register, but your access to certain services, such as home visits, may be limited.
To update your phone number or email address, we'll send a verification link to your current number/email address. If you no longer have access to this number/email, then you'll need to verify your identity in person with a valid ID.
You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
